ABSTRACT

In this article, we summarize clinical features and management of 3 cases with pathology confirmed granulomatous hypophysitis, to improve knowledge and provide treatment guidance in this regards. Patients presented with headache, blurred vision and central diabetes insipidus, respectively. The imaging manifestations were sellar area or pituitary stalk occupying lesions, and the nature was undetermined. The symptoms were not completely relieved after subtotal resection or biopsy. The symptoms could be controlled by high dose glucocorticoid, but relapsed after reduction. Two patients were treated with glucocorticoid combined with azathioprine. One case had no recurrence after withdrawal of the drug and 1 patient was still in treatment. Therefore, for the patients whose symptoms are still not relieved after operation and recurred after glucocorticoid reduction, the scheme of high-dose glucocorticoid combined with immunosuppressant azathioprine can be considered, which may achieve satisfactory curative effect and prognosis.

ABSTRACT

Objective To analyze the related factors of postoperative complications in elderly patients with colorectal cancer (≥80 years old) and to explore its prevention and treatment strategies. Methods A total of 86 patients with advanced colorectal cancer in Shanxi Dayi Hospital from January 2012 to January 2018 were selected, which were divided into the laparosopic group (35 cases) and the open group (51 cases) according to the operation methods. The clinical data, surgical methods, postoperative complications and related factors of the patients were retrospectively analyzed. Results The amount of bleeding, postoperative anal exhaust time and hospitalization time in the laparoscopic group were less than those in the open group, and the differences were statistically significant (t= 2.107, 3.631, 7.563, all P< 0.05). There was no significant difference in operative time between the laparoscopic group and the open group (t= 2.306, P>0.05). There was no significant difference in the number of postoperative complications for the patients whether cardiovascular and cerebrovascular diseases, diabetes and lung diseases were involved between the two groups (all P>0.05). The number of poor wound healing in the body mass index (BMI) ≥28 kg/m2 group (8 cases, 25.0 %) was more than that in the BMI < 28 kg/m2 group (3 cases, 5.6 %), and the difference was statistically significant (χ2= 5.179, P= 0.023). There was no statistical difference in the number of other complications between BMI ≥ 28 kg/m2 group and BMI < 28 kg/m2 group (all P> 0.05). The number of postoperative anastomotic fistula, pulmonary infection and poor wound healing in the laparoscopic group were all lower than those in the open group, and the differences were statistically significant (all P< 0.05). There were no statistically significant differences in postoperative cardiovascular and cerebrovascular accidents between the laparoscopic group and the open group (P= 0.543). Conclusions The elderly patients with colorectal cancer should attach great importance to perioperative diagnosis and treatment, so that preoperativecomplications can be effectively treated and controlled. Besides, it is necessary to give preference to laparoscopic surgery in the prevention of postoperative complications for elderly patients with colorectal cancer.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the diagnostic role of STAT6 immunohistochemistry in solitary fibrous tumors (SFT)/meningeal hemangiopericytomas (HPC).</p><p><b>METHOD</b>Evaluated the expression of STAT6, vimentin, CD34, EMA, PR, S-100, CD56, GFAP and Ki-67 in a cohort of 37 SFT/meningeal HPC, 30 meningiomas and 30 schwannomas by immunohistochemistry staining.</p><p><b>RESULTS</b>All SFT/meningeal HPC demonstrated nuclear positivity for STAT6, and the proportion of positive tumor cells ranged from 60% to 95%, with no significant difference cases.Vimentin was strongly positive in all cases. CD34, EMA and PR positivity was found in 32 cases, 1 case and 4 cases, respectively.S-100 protein, CD56 and GFAP were negative; Ki-67 labeling index was 1%-8%. However, the meningiomas and schwannomas were negative for STAT6.</p><p><b>CONCLUSIONS</b>STAT6 is a relatively specific biomarker for SFT/meningeal HPC, and may be used in the diagnosis and differential diagnosis of SFT/meningeal HPC, especially for the atypical cases, and allows the precise pathologic diagnosis of SFT/meningeal HPC.</p>

ABSTRACT

Diabetic retinopathy (DR) is one of the most common microvascular complications of diabetes,the incidence increased year by year,so the research of pathogenesis and interventions of DR is in the ascendant.The mechanism of DR mainly include glucose toxicity products and its effects on cell signaling pathways,leading to vascular disease and neuropathy.Research shows that,DR may occur with actived polyol pathway,increased,oxidative stress,the formation of advanced glycation end products (AGEs),protein kinase C pathway and vascular endothelial growth factor (VEGF)expression.db / db mice as an animal model of spontaneous type 2 diabetes,which is very similar to human type 2 diabetes,is more widely used in studies of DR.This article reviews some progresses of db / db mouse in the DR,and provide clues to further elucidate the pathogenesis of DR and development effective preventive measures.

ABSTRACT

Objective: To study the characteristics of P300 and topographic dist ribution mapping in obsessive-compulsive patients. Methods: The P300 and topog raphic distribution mapping were recorded in 36 patients, using a Bneuro Galileoinstrument. Results: Compared with normal subjects, the wa ve patterns of obsessi ve-compulsive patients were unstable; the frontal wave variation and dissymmetry between the two sides was 63.9%; the N2 and P3 latency was prolonged; the P2 an d P3 amplitude was decreased; the P3 topographic distribution mapping was uneven ly distributed, the normal high amplitude in the parietal region was absent. For the patient group, energy levels below grade 5 in the left brain area, the fron tal area, and for both were 42.3%, 30.3%, 15.15% respectively. Conclu sion: P300 and topographic distribution might be served as an objective index for reflectin g cognitive activity in obsessive-compulsive patients.
